The crash occurred on Kuhio Highway near mile marker four at around 2:47 p.m. when a Ford Explorer heading north crossed the centerline and crashed into a Dodge pick-up truck.
The decedent, Laurey Fernandez, 56, of Anahola was a passenger in the Explorer along with four others plus the driver.
The passengers of the Explorer were taken to Wilcox Hospital for treatment along with the driver of the pick-up truck.
The passenger in the pick-up truck was unharmed.
Kuhio Highway was closed in both directions after the crash. Bypass roads were opened around 3:20 p.m. and traffic moved slowly along these roadways.
Kuhio Highway was re-opened around 5:40 p.m.
Fernandez is the sixth traffic fatality on Kauai this year.
