Five people were killed when the Blue Hawaiian helicopter crashed during a scenic tour.
The report from the National Transportation Safety Board includes several eyewitness statements. One witness saw the helicopter descending from the ridgeline, and told investigators that he observed pieces falling from the helicopter. Another witness reported that the helicopter went straight down and impacted the ground sideways.
One witness also told investigators that the crash occurred during a heavy rain squall.
